Job Summary
Conduct one-on-one patient care and patient/family teaching in the home setting while performing patient assessments. Collaborate with the care team to develop and implement a Plan of Care, maintain regular communications with clinical management, and ensure timely, accurate documentation. Requires current RN license, home health/OASIS experience, and valid driver's license. Full-time position in Fulton County with per visit rates ranging from $50 to $135. Required Qualifications
- Graduate of an accredited Registered Nurse educational program
- Current Registered Nurse (RN) license within the state of practice
- Home Health/Oasis experience
- Current CPR certification
- Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
- Strong initiative, follow through, process orientation, attention-to-detail
- Reliable transportation
- Valid driver's license
- Current automobile insurance
