Job Summary
Provide home health nursing visits to adult and geriatric clients in designated North Carolina territories (Lexington, Thomasville, Denton) per individualized care plans. Perform admissions, administer medications, wound care, treatments, and procedures; monitor client conditions and report changes to the Clinical or Client Services Manager; follow up on and document doctors’ orders; conduct client assessments; engage in case management and coordination; accurately document observations and interventions using an EMR touchpad tablet. Travel to assigned cases as needed; flexible schedules and 1:1 visits emphasized.
Required Qualifications
- A current license as a Registered Nurse in North Carolina.
- A minimum of one year of recent, verifiable clinical experience.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
