Job Summary
Conduct comprehensive in-home assessments, develop individualized care plans, and deliver skilled nursing care including medication management and treatments. Serve as the clinical lead and case manager, coordinating care across disciplines to reduce hospitalizations and support patient independence. Monitor progress, educate families, and manage discharge planning while maintaining accurate EMR documentation. Supervise Home Health Aides as needed and participate in on-call rotation. This field-based role requires an active RN license with one year of experience, strong clinical judgment, and proficiency with Homecare Homebase.
Required Qualifications
- Active RN license (or compact eligibility)
- CPR certification
- Graduate of an accredited nursing program
- Minimum one (1) year RN experience
- Strong clinical judgment and ability to think independently
- Excellent communication skills and patient-centered approach
- Ability to manage a caseload and prioritize effectively
- Proficiency with EMR systems
- Valid driver's license and reliable transportation
Desired Qualifications
- Experience in home health or post-acute care
- Experience with case management and interdisciplinary coordination
- BSN
- Familiarity with Homecare Homebase (HCHB)
