Job Summary
Provide direct patient care in a home health setting as a Registered Nurse (RN) – Home Health Field Nurse. Responsibilities include performing evaluations of patients' functional status (muscle function, endurance, visual coordination, ADL/IADL performance), assessing cognitive, psychological and motor aspects, developing and revising individualized intervention plans, evaluating home environments for hazards, identifying equipment and environmental adaptations, participating with nursing in multidisciplinary care planning, educating patients and families in rehabilitative care to promote health and independent living, and accurately documenting observations and interventions.
Required Qualifications
- Valid nursing license in the state of California
- Minimum 1 year of experience as an RN in Home Health
- Proficient in taking and interpreting vital signs
- Full time / Part time position
