Perform visits and coordinate client care while serving as the point of contact for all disciplines involved in patient care. Oversee visit frequency for each episode, consult with physicians and the office regarding care details, and perform accurate OASIS collection. Ensure the medication profile remains current, report lab values to the physician timely, and attend weekly case conferences and monthly case manager meetings. Requires current RN licensure, at least one year of clinical experience, a valid driver's license, and automobile liability insurance.
