Coordinate client care and serve as the point of contact for all disciplines involved in patient visits. Perform accurate OASIS collection, maintain current medication profiles, and ensure timely lab value reporting to the physician. Consult with the physician and office regarding patient care details, oversee visit frequency for episodes, and attend weekly case conferences and monthly case manager meetings. Requires current RN licensure, one year of clinical experience, and a valid driver's license with insurance.
