Job Summary
Provide home health nursing care and coordinate patient care across disciplines. Conduct visits, manage the frequency of visits for episodes, consult with physicians and office staff on patient care details, perform accurate OASIS data collection, maintain current medication profiles, and ensure lab values are reported timely. Participate in weekly case conferences and monthly case manager meetings to support continuity of care.
Required Qualifications
- Be currently licensed as a Registered Nurse (RN) in the state of employment
- A minimum of one year of clinical experience is preferred
- Possess a valid state driver’s license and automobile liability insurance
- Demonstrate knowledge and skill in current nursing practice
