Job Summary
Perform comprehensive patient assessments, including OASIS assessments at required intervals, and develop individualized plans of care as patient needs evolve. Deliver skilled nursing services and treatments while counseling patients and families on disease processes, self-care techniques, and prevention strategies. Collaborate with the interdisciplinary team to ensure seamless, holistic care for patients in their homes across Pinellas county. Required Qualifications
- Graduate of an approved nursing program
- Active Florida RN license
- Minimum of 1 year of direct patient care experience
- Willingness to participate in weekend and holiday on-call rotations
Desired Qualifications
- BSN or MSN degree
- Prior home health RN experience
- Experience using Homecare Homebase (HCHB) EMR
- Strong assessment skills
- Strong critical thinking skills
- Strong patient education skills
