Job Summary
Registered Nurse (RN) – Home Health in Easton, PA with CarePine. Full-time role delivering personalized, one-on-one home health care: conducting initial assessments, developing and evolving patient-centric care plans, administering treatments and medications, providing health education to patients and families, coordinating seamless transitions with discharge planning, serving as case manager when designated, maintaining accurate clinical documentation, collaborating with physicians and care coordinators, and participating in 24/7 on-call coverage. Requires an accredited nursing program graduate, active RN license in PA, CPR certification, valid driver’s license, and 1–2 years of acute care (home health preferred). Skills include strong communication, critical thinking, clinical documentation, patient education, and teamwork with interdisciplinary staff. Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in the state
- current CPR certification
- valid driver’s license
- 1–2 years of acute care experience; home health care experience preferred

- Capable of lifting, positioning, and transferring patients
