Job Summary
Provide direct patient care in a home setting as an RN in Home Health, conducting functional and cognitive assessments, developing and revising individualized care plans, evaluating home environments for safety, identifying equipment and environmental adaptations, educating patients/families on rehabilitative care, documenting observations and interventions, and collaborating with the multidisciplinary care team to promote clients' health, safety and independence. Requires valid California nursing license, minimum 1 year home health experience, and ability to take/interpret vital signs; position available as either full-time or part-time.
Required Qualifications
- Valid nursing license in the state of California
- Minimum 1 year of experience as an RN in Home Health
- Proficient in taking and interpreting vital signs
