Job Summary
Conduct comprehensive nursing assessments, medication management, and documentation for patients in home health and hospice settings. Coordinate the implementation of individual care plans, lead interdisciplinary teams, and provide education and counseling to patients and families. Manage on-call rotations including weekly and weekend shifts, while maintaining infection control protocols and responding to patient calls. Participate in quality assurance programs, policy development, and in-service training to ensure compliance with professional standards. Requires a Wisconsin RN license with 2+ years in medical surgical, home health, hospice, or long-term care.

- On-Call Requirements: 1-Week On-Call
- On-Call Requirements: 24-Hour Weekend On-Call every 5th or 6th weekend
- Travel within a one-hour radius
- Company car provided for employees working 0.6 FTE or more
Desired Qualifications
- At least 2 years of experience in Medical Surgical, Home Health, Hospice or Long-Term care nursing
- Certified in Gerontology
- Certified in Wound Care
