Job Summary
Assess patient conditions, medical needs, and overall well-being during each visit. Create and modify care plans by actively shaping treatment based on evolving conditions. Teach patients medication guidance and wound care to support their independence at home. Communicate with providers and families to coordinate care when changes occur. Document visit notes accurately for compliance and continuity. Required Qualifications
- Active Registered Nurse (RN) license in Massachusetts
- One year of nursing experience
