Job Summary
We are seeking a skilled and dedicated Registered Nurse (RN) to provide professional nursing care to patients in a home health setting. Responsibilities include conducting in-home patient assessments, providing skilled nursing care such as wound care and medication management, monitoring patient conditions, documenting changes, and educating patients and caregivers. The role requires collaboration with interdisciplinary teams, ensuring compliance with regulations, and maintaining accurate patient records. Applicants must hold an active RN license in Indiana, have at least one year of nursing experience, CPR/BLS certification, excellent communication skills, and the ability to work independently.
Required Qualifications
- Active RN license in Indiana
- Minimum of one year of nursing experience

- Excellent communication, organizational, and interpersonal skills
- Ability to work independently and manage multiple patients efficiently
- Valid driver’s license and reliable transportation for home visits
