Job Summary
Registered Nurse (RN) - Home Health providing one-on-one patient care in patients' homes with autonomy and flexible scheduling. Responsibilities include conducting initial and ongoing patient assessments, developing and revising care plans, administering prescribed medications and treatments, counseling patients and families, coordinating with physicians and community health resources, documenting clinical notes, and participating in on-call duties. Must be licensed as an RN in the state, hold current CPR certification, possess a valid driver's license, and be able to lift and assist patients. Strong communication, organization, and problem-solving skills are essential; professional nursing judgment is used to plan and direct home care services with emphasis on community health education and independent practice in the field.
