Registered Nurse (RN) Home Health

Collaborate with physicians to address medication needs and adjust care plans, while leading and supervising LPNs, aides, and therapy staff to ensure high-standard patient care. Complete OASIS recertifications, discharge documents, and other necessary documentation within the EMR system. Actively monitor patient conditions to prevent re-hospitalization and provide education and counseling to families regarding disease management and injury recovery. Attend orientation sessions, team meetings, and Continuing Education Units to maintain professional growth. This role requires a current Ohio RN license, CPR certification, reliable auto and insurance, and the ability to work independently with minimal supervision.
