Job Summary
Registered Nurse (RN) – Home Health: Provide patient-centered nursing care in patients' homes. Collaborate with the physician on medication needs and changes to the plan of care; oversee and coordinate care with LPNs, aides, and therapy staff. Complete OASIS, recertifications, discharges, and ROC documentation within an EMR system. Contribute to reducing hospital readmissions by actively monitoring the patient’s plan of care and changes in condition. Attend orientation, meetings, and CEUs; utilize AI-powered tools to enhance operational efficiency and clinical decision-making.
