Job Summary
Registered Nurse Home Health providing 1:1 patient care in-home within Des Plaines and surrounding IL areas. Responsibilities include initial evaluations, ongoing assessment of physical, psychosocial, and environmental needs, planning, coordinating, implementing and evaluating the plan of care, communicating changes to the Clinical Supervisor and team, and traveling within the defined territory to deliver high-quality home-based nursing care. Requires current RN license, at least 1 year of clinical nursing experience, ability to travel up to 50% within the branch/office area, and willingness to work in a home-health setting. Associates or Bachelor’s degree in nursing preferred; HomeHealth/HomeCareBase experience preferred. Compensation base salary $88,000-$95,000 annually; comprehensive benefits and equal opportunity employer.
Required Qualifications
- Current, unrestricted state RN license
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Associates Degree or Bachelor’s Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with HomeCare HomeBase is preferred
