Job Summary
Registered Nurse Home Health responsible for evaluating and planning patient care in home settings, performing initial evaluations and follow-ups, coordinating with the Interdisciplinary Team, and communicating changes in health status to supervising clinicians. Maintains patient privacy per HIPAA, documents care plans and nursing activities, and delivers 1:1 patient care with autonomy. Travel within branch/office coverage area up to 50% is required; flexible schedules and comprehensive benefits are provided.
Required Qualifications
- Current, unrestricted state RN license
- 1+ year nursing experience in a clinical care setting
- Able and willing to travel within branch/office coverage area (up to 50%)
- Valid driver’s license and reliable vehicle
