Job Summary
Assess assigned case loads of home health care patients to identify physical, psychosocial, and environmental needs through documentation, clinical records, and on-site evaluations. Develop and implement plans of treatment by making initial evaluations, re-evaluating nursing needs during each visit, and revising care based on identified issues. Coordinate care by communicating significant findings, problems, and changes in health status or unsafe facility conditions to clinical supervisors, physicians, and other personnel. Maintain patient and staff privacy in accordance with HIPAA while ensuring all activities align with Elara Caring's board vision. Requires current unrestricted state RN license, one year clinical experience, and ability to travel 50% with a dependable vehicle. Preferred qualifications include a Bachelor's degree, previous home health or hospice experience, and familiarity with HomeCare HomeBase.
Required Qualifications
- Current, unrestricted state RN licensee
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
Desired Qualifications
- Associate Degree or Bachelor's Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with HomeCare HomeBase is preferred
