Job Summary
Assess assigned case loads of home health care patients to identify physical, psychosocial, and environmental needs through documentation, clinical records, and on-site evaluations. Assume primary responsibility for caseloads by assessing, planning, coordinating, implementing, and evaluating treatment plans. Make initial evaluations and re-evaluate nursing needs during each visit, while communicating significant findings, problems, and unsafe facility conditions to the Clinical Supervisor, physician, and other personnel. Revisit the plan of care in response to identified issues and notify relevant team members. Maintain patient and staff privacy and confidentiality pursuant to HIPAA. Requires current, unrestricted state RN licensure, one year of clinical experience, and ability to travel 50% within the coverage area.
Required Qualifications
- Current, unrestricted state RN licensee
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Dependable vehicle
- Valid driver's license
- Current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
Desired Qualifications
- Associate Degree or Bachelor's Degree in Nursing
- Previous Home Health or Hospice Experience
- Previous experience with HomeCare HomeBase
