Registered Nurse RN Home Health

Assess assigned case loads of home health care patients to identify physical, psychosocial, and environmental needs through documentation, clinical records, and interdisciplinary team reports. Assume primary responsibility for the plan of treatment by assessing, planning, coordinating, implementing, and evaluating care during each visit. Make initial evaluations and re-evaluate nursing needs, then communicate significant findings, problems, and unsafe facility conditions to the Clinical Supervisor, physician, and other personnel. Revisit the plan of care in response to identified issues and notify relevant team members. Maintain patient and staff privacy and confidentiality pursuant to HIPAA. Requires current, unrestricted state RN licensure, 1+ year of clinical experience, and 50% travel within the branch coverage area.
