Job Summary
Provide high-quality nursing care to patients in their homes; develop and implement individualized patient care plans; administer medications and treatments as prescribed; complete and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition within defined agency time frames; must hold MA RN license and have 1-2 years RN experience with at least 1 year in home health; demonstrate strong clinical assessment, communication skills, and reliable transportation to travel within the service area; flexible scheduling and supportive team environment.
Required Qualifications
- Valid Registered Nurse license in Massachusetts
- 1-2 years of experience as an Registered Nurse
- 1 year of experience in home health care required
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Reliable transportation and willingness to travel within the service area.
