Job Summary
Conduct home health visits for adult and geriatric patients in senior living communities across Forsyth and Guilford counties. Administer medication, perform wound care, and execute treatments while monitoring client conditions and reporting changes to managers. Follow designated care plans, perform assessments, and accurately document observations, interventions, and evaluations using a touch pad tablet. Manage case coordination and doctor's orders within designated geographic territories. Requires a current North Carolina RN license with at least one year of clinical experience.
Required Qualifications
- A current license as a Registered Nurse in North Carolina
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home health or senior living community experience
- Prior experience with electronic medical records (EMR)
