Job Summary
Conduct head-to-toe assessments, triage urgent clinical situations, and manage acute and post-acute care for home health clients. Perform medication reconciliation, wound care, and safety monitoring while documenting visits efficiently. Triage and manage urgent clinical situations independently in the home setting, utilizing strong problem-solving skills in unpredictable environments. Requires a current Colorado nursing license, one year of adult/geriatric experience, and ability to work independently with real-time clinical decision-making. Flexible scheduling with weekly pay of $55-60 per point and mileage reimbursement.
Required Qualifications
- Current nursing license in good standing in the state of Colorado
- Graduation from an accredited and approved nursing program
- At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings)
- Ability to work independently while knowing when to escalate concerns
- Comfortable making real-time clinical decisions without immediate on-site support
- Strong problem-solving skills in unpredictable home environments
- Ability to manage a dynamic visit schedule and prioritize urgent visits
- Efficient with point-of-care documentation and technology
- Strong attention to detail while maintaining productivity expectations
- Comfortable providing care in varied home environments
- Flexible and adaptable to changing patient needs and schedules
- Demonstrates cultural sensitivity and respect for patient autonomy
Desired Qualifications
- Prior Medicare Certified Home Health experience
