Job Summary
Assess assigned home health care patients and families to identify physical, psychosocial, and environmental needs through documentation, clinical records, and interdisciplinary team reports. Develop and implement plans of treatment by making initial evaluations, revising care based on identified issues, and conducting follow-up visits according to patient and facility needs. Communicate significant findings, health condition changes, or unsafe facility conditions to the Clinical Supervisor, physician, and other personnel involved in care. Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule while ensuring all activities align with Elara Caring's board vision. Requires current unrestricted state RN license, 1+ year clinical experience, and 50% travel within the DeKalb, Genoa, and Waterman, IL markets.
Required Qualifications
- Current, unrestricted state RN licensee
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
Desired Qualifications
- Associate Degree or Bachelor's Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with HomeCare HomeBase is preferred
