Job Summary
Home Health RN provides home-based care to patients, supervises LPNs/CNAs/aides, conducts assessments, documents progress, and coordinates discharge. Responsibilities include educating patients and families on the plan of care, medications, therapy, diet, and home safety; collaborating with physicians; and ensuring goals are met. Requirements include an accredited nursing program graduate with an active Wisconsin RN license, CPR, and at least 1 year of nursing experience (preferably in home health).
Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in Wisconsin
- CPR certification
- Minimum of 1 year nursing experience, ideally in home healthcare
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing specialty
- Strong communication skills and compassion
- Ability to educate patients on plan of care, medications, therapy, diet and exercise
- Coach family members on patient’s plan of care and home safety
- Experience with coordinating discharge from services
