Job Summary
Conduct home care visits by performing assessments, executing physician treatment plans, and counseling patients and families on healthcare needs. Manage assigned patient populations through clinical decision-making while supervising LPNs and coordinating equipment and supplies. Complete accurate clinical documentation and participate in Quality and Performance Improvement (QAPI) and emergency preparedness programs. Work part-time, approximately 20 hours per week, Monday through Friday, with flexible scheduling and potential for additional hours. Requires an active MN RN license, one year of home health experience, and CPR/BLS certification.
Required Qualifications
- An active RN license in the states of MN
- One year of experience in Home Health
- Current CPR/ BLS
- Must have reliable transportation for patient visits including insurance
