Job Summary
Admit patients requiring skilled nursing by performing thorough physical, social, emotional, and safety assessments. Develop individualized care plans, obtain physician orders, and evaluate patient progress toward goals while revising treatment plans as needed. Educate patients and families, manage secondary symptoms and comorbidities, and arrange for supplies and durable medical equipment. Collaborate with interdisciplinary team members, identify discharge planning needs, and make referrals for community resources. Supervise Home Health Aides, ensure infection control compliance, and complete accurate documentation within 24 hours. Participate in on-call rotations and quality improvement activities while adhering to Medicare, Medicaid, and HIPAA regulations.
Required Qualifications
- Graduation from an NLN accredited school of nursing
- At least one year clinical experience
- Current license to practice as a registered nurse in Connecticut
- Possesses and maintains current CPR certification
- Excellent observation, problem-solving, verbal and written communication skills
- Nursing skills per competency checklist
- Self-directed with the ability to work with little supervision
- Flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team
- Shows ability to organize and prioritize workload independently
- Current valid driver's license
- Able to walk and/or stand for prolonged periods of time
- Able to lift, position or transfer patients
- Able to lift supplies and equipment
- Able to reach, stoop, bend, kneel or crouch
- Visual acuity and hearing to perform required nursing skills
- Able to participate in agency weekend and on-call program
