Job Summary
Assess and authorize services for older adults and individuals with disabilities by determining eligibility and establishing care plans per funding regulations. Collaborate with care management teams to define service frequency, scope, and duration while promoting cost-effective substitutions. Maintain records, prepare reports, and provide consultation to providers and community agencies on current policies and standards. Lead assessment coordination, participate in interdisciplinary efforts, and conduct training workshops as needed. Requires current Massachusetts RN license, two years of clinical experience, and proficiency in Microsoft Office. Full-time role with 35-hour week, hybrid environment, and home-based visits.

- Excellent interpersonal and conflict resolution skills
- Excellent organizational skills and attention to detail
- Strong analytical and problem-solving skills
- Proficient with Microsoft Office Suite or related software
- Considerable knowledge of developing assessment care plans
- Extensive knowledge of the medical, emotional and social needs of elders and people living with disabilities
- Associate degree in Nursing or related field
- Current Massachusetts RN License
- Two years' clinical/community-based experience
- Ability to travel and reliable means of transportation
- Must have personal computer with secure internet access at home
- Must be able to lift-up to 25 pounds at times
