Job Summary
Registered Nurse (RN) responsible for creating and overseeing Plans of Care, delivering direct skilled nursing services, supervising caregivers, and ensuring compliance with federal/state regulations within a home-based care setting. key duties include administering Care Plans in coordination with the Director of Nursing and care team, conducting on-site orientations and regular supervision of home care aides, updating and documenting client care plans, performing nursing assessments, participating in audits, and traveling to patient homes as needed (travel 10-20%). Requirements include NY RN licensure, graduation from an approved nursing program (Diploma/BSN/MSN), valid driver’s license with reliable transportation, strong communication and organizational skills, and CPR certification (bilingual preferred).
Required Qualifications
- Graduate of an approved school of professional nursing with a Diploma, Baccalaureate, or Master’s Degree
- Currently licensed and registered as a Registered Nurse in the practicing state (NYS required)
- Must be a licensed driver with independent transportation and current insurance coverage
- Driver’s license & vehicle
- Bilingual preferred
