Job Summary
Registered Nurse (RN) in-home clinical lead for ComForCare Home Care. You will conduct in-home patient assessments, develop and oversee individualized care plans, supervise CNAs/GNAs/HHAs/Med Techs, coordinate follow-up care, communicate with families, physicians, and care teams, ensure safe, compliant, high-quality care delivery, maintain clinical documentation, and provide guidance and education to caregivers. Requires active Maryland RN license, CPR/First Aid certification, and reliable transportation for home visits. Flexible scheduling and one-on-one patient care in the home; immediate openings available.
Required Qualifications
- Active Maryland RN license (or compact license eligible in MD)
- CPR/First Aid certification
- Reliable transportation for home visits
- Strong clinical assessment and communication skills
- Experience in home care, hospice, or geriatrics preferred
