Job Summary
Conduct comprehensive nursing assessments during home visits, develop individualized care plans, and administer medications, treatments, and wound care as ordered. Educate clients and families on disease management, medications, safety, and self-care while monitoring progress and identifying clinical changes. Coordinate services with physicians, therapists, and social workers, and participate in office-based care conferences and quality improvement activities. Complete accurate clinical documentation and ensure compliance with agency policies and state regulations. Requires current unrestricted RN license, four years clinical experience, BLS certification, and a valid driver's license. Offers flexible scheduling, weekly pay, and 401K matching within a supportive home care team environment.
Required Qualifications
- Current, unrestricted Registered Nurse (RN) license
- Minimum of four (4) years of clinical nursing experience
- Excellent assessment, critical thinking, organizational and communication skills
- Ability to work independently while collaborating with an interdisciplinary team
- Technologically savvy, proven ability to learn and master new software and use of variety of tech equipment
- Current TB Test
- BLS Certification (American Heart Association)
- Valid driver's license
- Reliable transportation
- Current automobile insurance
