Job Summary
Determine the appropriate level of care and skilled needs for patients based on Conditions of Participation. Perform comprehensive initial and follow-up assessments of physical, psychological, and environmental parameters, then formulate individualized plans of care according to physician orders. Deliver patient care using a patient/family-centered approach while providing education on end-of-life issues, palliative care, and discharge planning. Relay significant changes in patient status to the physician and interdisciplinary team promptly. Collaborate with the care team to modify education based on patient goals and promote patient/caregiver autonomy. Requires weekend shift (Fri-Sun 8 AM - 8:30 PM) and ability to travel to patient assignments.
Required Qualifications
- Specialized Diploma NLN approved/accredited nursing program
- 1 year recent experience providing direct care in a healthcare setting
- Knowledge of patient education techniques and principles
- Ability to take initiative and function independently without direct supervision and as a member of the care team
- Ability to travel to patient care assignments
- Knowledge of Conditions of Participation
- Knowledge of and skill in completing regulatory data set collection with a high level of accuracy
- Ability to facilitate difficult and complex communication related to goals of care
- Highly skilled in verbal and non-verbal communication
- Exhibit excellent time management and organization skills
- BLS - Basic Life Support AHA - American Heart Association Within 30 Days
- RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire
- DL - Driver's License_PA - State of Pennsylvania Upon Hire
- DL - Driver's License_NJ - State of New Jersey Upon Hire
- Lift and carry 40-50 lbs
- Frequent to continuous standing/walking
- Patient transporters can walk 8-10 miles per shift
- Activities specific to their role including kneeling, bending, squatting and performing CPR
- Work Schedule: Fri, Sat, Sun 8 AM - 8:30 PM (48 out of 52 weekends)
Desired Qualifications
- Associate's Degree Nursing or Bachelor's Degree Nursing
- 1 year experience as a home health or hospice Registered Nurse
- Knowledge and ability to navigate multiple computer applications and EMR platforms
- AMB-BC- Ambulatory Care Nursing-Board Certified - American Nurses Credentialing Center within 3 Years
- CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association within 3 Years
