Registered Nurse (RN) Home Care Admissions Nurse - Weekend $10,000 Sign On Bonus

Job Summary
Performs comprehensive subjective and objective initial and follow-up assessments of patients status and formulates an individualized plan of care according to physician orders; delivers patient care based on the medical plan of treatment; provides patient and family education on end-of-life issues, chronic disease management, pain and symptom control, home care, hospice, and discharge planning; supports family-centered care and evaluates education effectiveness. The role requires travel to patient care assignments, coordination with the care team, and adherence to Home Health Conditions of Participation. Weekend daytime schedule with a $10,000 sign-on bonus.
Required Qualifications
- Bachelor’s Degree Nursing or Associate’s Degree Nursing with 2 years experience in the home care setting
- 1 year recent experience providing direct care in a healthcare setting
- 1 year Home Health experience in OASIS data set collection
- Knowledge of patient education techniques and principles
- Ability to travel to patient care assignments
- American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
- RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire
- DL - Driver's License_PA - State of Pennsylvania Upon Hire
- AMB-BC- Ambulatory Care Nursing ANCC - State of Pennsylvania within 3 Years
