Job Summary
Execute physician-prescribed plans of care for patients residing in homes across Arizona, Colorado, Kentucky, Missouri, Nevada, New Mexico, Tennessee, Texas, and Utah. Complete compliant documentation of care provided in the system of record, manage and administer prescribed medication, treatments, and therapies, and perform various activities associated with daily living. Monitor for changes in the patient's condition, such as weight loss/gain, self-care abilities, and indicators of disease progression. Communicate with family members, physicians, case managers, and other prescribed individuals regarding the patient's health. Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants while performing other personal care services as necessary to meet the patient's needs. This 1099 contract role requires a current RN license, 1+ years of experience, CPR certification, TB testing, and dependable transportation.
Required Qualifications
- Current State or Compact License as a Registered Nurse
- 1+ years of experience as an RN in a clinical or home health setting
- Current CPR certification
- Willing to get TB tested
- Maintain required insurance
- Dependable transportation
- Willing to commute to the patient's home
