Job Summary
Per diem Registered Nurse (RN) for home health in Gilbertsville and surrounding areas. Responsibilities include initial assessments, developing and evolving patient-centered care plans, ongoing nursing assessments, administering treatments and medications, health education for patients and families, discharge planning, potential Case Manager duties, ensuring accurate documentation, and collaboration with physicians and care teams. Additional duties include on-call coverage for 24/7 care continuity and supervising home health aides biweekly. Requirements: RN licensure in PA, CPR, driver’s license, 1–2 years of acute care experience; associate or BSN preferred. Eligible for CarePine Home Health services; Medicare/Medicaid-certified provider.
Required Qualifications
