Lead interdisciplinary rounds to facilitate appropriateness of care and smooth transitions across the continuum for patients with renal and gastroenterological illnesses, including End Stage Renal Disease, Peritoneal Dialysis, and acute kidney injury. Effectively prioritize clinical, social, and safety issues while applying the nursing process to meet the clinical, psychosocial, and spiritual needs of patients and families. Delegate care appropriately while retaining accountability for quality, and actively participate in unit-based self-care and shared governance activities to support continuous improvement. Work within a 39-bed medical/surgical unit designated for renal specialty care, providing physical, emotional, and spiritual support to patients and families.
