Registered Nurse (RN) Full-time

Lead and coordinate patient care in collaboration with physicians while performing assessments, including OASIS, and developing individualized care plans. Supervise and support licensed practical nurses and home health aides during visits, communicating closely with families and interdisciplinary team members. Maintain an RN license in the State of Ohio with home health experience preferred, alongside strong organization, compassion, and reliable transportation.
