Job Summary
Conduct client assessments to develop individual care plans while providing primary care in collaboration with healthcare professionals. Manage complex wounds, IV therapy, dialysis, and chronic disease education, alongside medication administration and phlebotomy. Deliver palliative and end-of-life care, rehabilitative programs, and community outreach across clients' homes, clinics, and virtually. Requires a current Registered Nurse license, personal vehicle, driver's license, and proof of insurance. Includes evening and weekend rotations with on-call responsibilities.
Required Qualifications
- Must be licensed as a Registered Nurse in the province of practice
- Must enjoy driving
- Must have a personal vehicle
- Must possess both a current driver's license and proof of vehicle insurance
- A satisfactory Criminal Records Check is required
- A Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary
- Must be able to wear Personal Protective Equipment (PPE)
- This role requires a detail-oriented approach in a dynamic environment
- This role requires physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics
- Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day
- Some tasks require fine hand movements
