Job Summary
Provide case management, coordination, and supervision of professional services to clients using nursing theory and the Nurse Practice Act. Review client referrals, respond to inquiries, and perform assessments based on agency standards to ensure effective home care. Direct, plan, and initiate independent action in in-home care situations while adhering to agency policies and community standards. Perform other duties as assigned by the Director of Clinical Services. Full-time Monday through Friday with occasional weekends.
Required Qualifications
- Graduate of an accredited school of professional nursing program
- Current license as a RN in the state(s) of Colorado
- Minimum of two (2) years of nursing experience
- CPR certified
Desired Qualifications
- Baccalaureate degree preferred
- Home care experience preferred
- Experience with a combination of long-term care dementia care or acute care
