Job Summary
Conduct case management, coordinate, and supervise professional and supportive services for clients using nursing theory and process in accordance with the Nurse Practice Act. Perform client assessments based on agency standards, review referral information, and respond to inquiries as appropriate. Direct, plan, and initiate independent action in-home care situations while adhering to agency policies, procedures, and professional standards. Provide other duties as assigned by the Director of Clinical Services. This full-time role in Montrose, CO, supports a Medicare/Medicaid-certified home health agency serving older adults in Delta, Montrose, and Ouray counties.
Required Qualifications
- Graduate of an accredited school of professional nursing program
- Current license as a RN in the state(s) of Colorado
- Minimum of two (2) years of nursing experience
- CPR certified
Desired Qualifications
- Baccalaureate degree preferred
- Home care experience preferred
