Job Summary
Provide case management, coordinate supervision, and implement supportive services to clients using nursing theory and the Nurse Practice Act. Review referral information, respond to inquiries, and perform client assessments based on agency standards. Direct, plan, and initiate appropriate action independently in-home care situations while adhering to agency policies and community standards. Perform other duties as assigned by the Director of Clinical Services. Full-time Monday through Friday, 8-hour shifts with occasional weekends.
Required Qualifications
- Graduate of an accredited school of professional nursing program
- Current license as a RN in the state(s) of Colorado
- Minimum of two (2) years of nursing experience
- CPR certified
Desired Qualifications
- Baccalaureate degree preferred
- Home care experience preferred
