Job Summary
Provide case management, coordination, and supervision of professional and supportive services to clients using nursing theory and the Nurse Practice Act. Review client referral information, respond to inquiries, and perform client assessments based on agency standards. Direct, plan, and initiate appropriate action independently in-home care situations while adhering to agency policies and professional standards. Duties include implementing services for older adults in the Delta, Montrose, and Ouray counties. Requires a Baccalaureate degree, current Colorado RN license, two years of nursing experience in long-term care dementia or acute care, and CPR certification. Full-time Monday through Friday schedule with occasional weekends.
Required Qualifications
- Graduate of an accredited school of professional nursing program
- Current license as a RN in the state(s) of Colorado
- Minimum of two (2) years of nursing experience
- CPR certified

- Baccalaureate degree preferred
- Home care experience preferred
