Job Summary
Provide case management, coordination, and supervision of professional services to clients using nursing theory and the Nurse Practice Act. Review referral information, respond to inquiries, and perform client assessments based on agency standards to ensure effective home care. Direct, plan, and initiate appropriate actions independently in in-home care situations while adhering to agency policies and community standards. Duties are performed as assigned by the Director of Clinical Services. Full-time Monday through Friday, 8-hour shifts with occasional weekends.
Required Qualifications
- Current license as a RN in the state(s) of Colorado
- Minimum of two (2) years of nursing experience, with a combination of long-term care dementia care or acute care
- CPR certified

- Home care experience
- Baccalaureate degree preferred
