Registered Nurse (RN) - Full Time

Conduct comprehensive in-home patient assessments, including OASIS assessments at start of care, recertification, and discharge. Develop and revise individualized plans of care while delivering skilled nursing services such as wound care, medication administration, and IV therapy. Educate patients and caregivers on disease processes and safety to support independence, then coordinate care with physicians, therapists, and interdisciplinary teams. Supervise LPNs/LVNs and home health aides with required clinical visits, documenting all care accurately in the electronic medical record. Ensure compliance with Medicare, Medicaid, HIPAA, and regulatory standards while managing caseloads independently across Waterbury, CT and Hamden, CT. Participate in case conferences and on-call rotation as assigned.
