Job Summary
Start of Care (SOC) visits for home health patients: conduct assessments, create individualized care plans, educate patients and families, document in EMR, and collaborate with a supportive team to ensure high-quality initial care and patient confidence. This RN role emphasizes autonomy, compassionate patient interaction, and delivering clear, proactive care from day one. License required in Michigan; travel between Saginaw/Midland/Bay City areas supported by mileage reimbursement. The position offers full benefits, flexible scheduling, and opportunities for growth.
Required Qualifications
- Active RN license in Michigan
- Home health experience is a plus — but we’ll train the right nurse
- Strong assessment skills and clinical judgment
- Reliable transportation and solid communication skills
