Job Summary
Provide Start of Care (SOC) visits as an RN in home health, performing assessments, creating individualized care plans, educating patients and families, and documenting in the EMR. Collaborate with a supportive team to ensure compassionate, patient-centered care from day one and help patients feel safe, empowered, and ready for their care journey. Demonstrates clinical excellence, autonomy, and strong communication to set the tone for optimal outcomes during home visits.
Required Qualifications
- Active RN license in Michigan
- Home health experience is a plus â but weâll train the right nurse
- Strong assessment skills and clinical judgment
- A warm, upbeat personality that helps patients feel instantly at ease
- Reliable transportation and solid communication skills
