Job Summary
Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Deliver skilled nursing interventions including wound care, infusions, catheter care, medication management, and chronic disease management tailored to individual goals. Educate patients and caregivers on symptom and medication management while monitoring progress to identify condition changes and prevent hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation supporting quality and compliance. Serve as the primary clinical partner for patients and families, promoting recovery, safety, and independence within the home. Monday-Friday schedule with on-call rotation; requires active RN license, one year clinical experience, driver's license, and reliable transportation.
