Job Summary
Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, then translate findings into patient-centered care plans. Deliver one-on-one skilled nursing interventions including wound care, infusions, catheter care, and medication management tailored to patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to identify early changes in condition. Coordinate care across providers to ensure continuity and prevent avoidable hospitalizations, completing timely documentation that supports quality and compliance. Work Monday through Friday with shared on-call rotation in Mobile, AL communities.
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- at least one (1) year of clinical nursing experience
- Is patient‐focused, organized, and motivated by delivering high-quality care
- Has a current driver's license
- Has reliable transportation
- Has auto insurance
- must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
