Job Summary
Assess behavioral health client nursing care needs and provide skilled nursing care and health monitoring services. Coordinate client care with treatment teams, counsel clients and families on illness prevention, and complete prior authorization medication forms. Plan and coordinate appointments for psychotropic injections, maintain case records, and oversee medication storage and hazardous waste management. For Medication-Assisted Treatment (MAT), screen patients for program fit, lead intake processes, and administer evidence-based assessments like COWS. Monitor lab tests, document client behaviors, and coordinate with prescribers and pharmacies to enhance program adherence. Maintain active DEA lists and complete external referrals for medical needs while promoting the MAT program to the community.
Required Qualifications
- Completion of a course of study approved by the New York State Education Department as qualifying for Registered Professional Nurse (RN)
- Possession of a New York State Registered Nurse's License at time of application
- Must be authorized to work in the United States without work sponsorship
- Must be able to stand and walk for prolonged periods of time
